Calibration Failure Log
Purpose: A thin, project-wide scar index for calibration design failures — cases where the mechanism behaved as specified and still produced misleading calibration data or false confidence. Short entries only. Depth lives on Talk, a page cal report, or a Decision Record — not duplicated here.

- The result was false or misleading for calibration (standing, dates, index membership, confidence).
- The lesson is reusable beyond a single content page.
