Canonical Question: When a calibration mechanism works exactly as designed but the design still produces false confidence, where is that lesson kept findable without building a third archive of every report?

Calibration Failure Log

Purpose: A thin, project-wide scar index for calibration design failures — cases where the mechanism behaved as specified and still produced misleading calibration data or false confidence. Short entries only. Depth lives on Talk, a page cal report, or a Decision Record — not duplicated here.

Filing rules (keep it thin)

File a scar here only if all of the following hold:

  1. The mechanism followed its design (or the documented procedure as written).
  2. The result was false or misleading for calibration (standing, dates, index membership, confidence).
  3. The lesson is reusable beyond a single content page.
  4. You can state the scar in a few lines and point elsewhere for depth.

Do not file here:

  • Software/config bugs → Known Site Issues & Fixes
  • One-page wrong claims or local structure NCR → that page’s report
  • Routine Development / Confirmation / pre-cal history → page Talk or Calibration Log
  • Open WIP incompleteness → breadcrumb on that page
  • Full governance rationale → Decision Record (add a one-line scar here only if useful as an index pointer)
