Scannable list of governance decisions by permanent ID. Does not hold full reasoning. Standard: Decision Records: Governance Memory. Maintenance rule: When you create or change a Decision Record, update this Index in the same edit — do not defer. IDs: sequential DR-NNNN, never reused. Index sorted by ID. Retroactive insertion of an earlier decision is allowed; note it in the row or Open Items if order vs chronology is confusing. First-version note: Column set and sort may evolve after more than the initial rows are in real use.

How to add an entry

Assign next sequential ID.
Write the full record on the owner home: preferably PageName/Decision Records, or Talk on Talk-only infrastructure pages — still register here.
Add one Index row in the same edit: ID, title, page(s), status, superseded-by, date, link.
Multi-page decisions: list all concerned pages; full text on the enforcing page; cross-link others.

Open items

Topic tags (Governance, Taxonomy, Calibration, …) for browse-by-theme — later.
Sharper threshold if overhead grows: prefer “cross-page consequences or rejected serious alternative” over filing every local preference — refine after more live use (Decision Records: Governance Memory).
Next framework DRs (static dates, report tiers, scar-index role, etc.) when written — append rows, do not reserve empty IDs.
