Decision Records: Governance Memory/Index

Breadcrumb (first-version note): This is the first version of the Index. Its structure — column choices, sort order, tagging — is expected to evolve once it's actually been used across more decisions than the five it currently holds.

Decision Records preserve governance history. They do not make decisions immutable; they make later recalibration traceable. This page is a scannable list only — it does not contain full reasoning. For the complete record behind any entry, follow the link in the Full Record column. See Decision Records: Governance Memory (this Index's parent page) for the full standard governing this system.

This index is manually maintained. When adding a new Decision Record, always update this page in the same edit — do not defer it.

Decision Records are assigned permanent, sequential IDs. The Index is maintained in ID order, which normally corresponds to chronological creation — but retroactive insertion of an earlier decision, discovered later, is anticipated and does not break this system.

IDs assigned retroactively, in document order. Anchor links in the Full Record column are placeholders pending confirmation of that page's actual section headings — verify before relying on them. Dissent for DR-0004 (Rejected) is preserved in full inside its source record, not restated in this table, per the standard's own guidance to keep the table clean and put reasoning in the record body.

How to Add an Entry

When a new Decision Record is created:

  1. Assign the next sequential ID.
  2. Write the full record on the appropriate `PageName/Decision Records` subpage, following Decision Records: Governance Memory's format.
  3. Add one row here: ID, title, page(s) concerned, status, superseded-by (if applicable), date, and a direct link to the full record.
  4. If the decision spans multiple pages, list all in "Page(s) Concerned," and cross-reference from each page's own subpage back to wherever the full record lives.

Open Items

  • Tags/categories for browsing (Governance, Templates, Taxonomy, Calibration, Infrastructure) — flagged as a future feature, not built now. Would let someone browse by topic instead of only chronology/ID.
  • Administrative overhead risk: as governance changes accumulate, creating a Decision Record + Index entry + Calibration Report + Calibration Log + Breadcrumb for every change may become tedious. A sharper threshold rule than the current trigger test (e.g. "only decisions with cross-page consequences require a Decision Record") may emerge with more real use — not solved now, flagged for future refinement.
  • Anchor links in the Full Record column need verification against the actual live headings on the source page once that page is retrofitted (see Decision Records: Governance Memory's open items).
